Compassionate support for you and your loved ones.

We understand that the need for palliative care can be an emotional time for families and that most people would prefer to spend their final days in the comfort of their own homes. It can be a very daunting time so we will work quickly to get a support plan in place that fulfills your loved one’s care needs, enabling them to spend more time with friends and family during this difficult time.

We want to ensure that your loved one is as comfortable as possible so your local branch team will assess their clinical need as well as preferences and interests, in order to match them with carers they feel at ease with and who can facilitate your loved one continuing with all the activities they enjoy for as long as possible.

What is Palliative Care at home?

Palliative care at home is a specialised approach to medical care provided to individuals with serious illnesses or life-limiting conditions within the comfort and familiarity of their own homes. The primary goal of palliative care is to enhance the quality of life for patients by addressing their physical, emotional, and spiritual needs, while also providing support and comfort to their families.

Everyone’s diagnosis is different, so each palliative care plan will be tailored specifically to your loved one’s needs and wishes. Their bespoke plan could cover:

  • Companionship

  • Personal care

  • Medication administration

  • Symptom management

  • Nursing care including PIC lines, intravenous injections etc.

Your local branch team will be on hand to liaise with other parties involved with your loved one’s care including doctors, hospitals and social workers to accommodate any additional needs that may arise.

Our approach is flexible, your care plan can start with companionship and assistance with daily tasks and evolve as your needs change. Our goal is to empower you to maintain your independence, ensuring your comfort and well-being throughout your journey.

  • We will work at your pace, but our skilled team can put together a care plan in as little as 24 hours. Call us today on 08000 622 622 to find out more.

  • Palliative care and end-of-life care share the common goal of providing comfort and support to individuals facing serious illnesses, but they differ in their focus and timing:

Palliative Care: Palliative care can begin at any stage of a serious illness, not just at the end of life. Its primary aim is to enhance the quality of life by addressing pain, symptoms, and emotional or social challenges. It can run alongside other treatments and focuses on improving well-being.

End-of-Life Care: End-of-life care, is specifically for individuals who are nearing the end of their life. It concentrates on providing comfort, dignity, and support in the final stages of a terminal illness. The focus is on ensuring a peaceful and pain-free transition in the final days or weeks of life.

  • Palliative care is often split into five stages and understanding these can help you to make decisions for your own care to ensure that you are receiving the right support at the right time.

Stage 1: Creating a plan - In the initial stage, a care plan is developed in collaboration with other medical professionals involved in your care.

Stage 2: Preparing emotionally - Depending on the level of care outlined in stage one, emotional support for you and your family will be arranged by your social worker, counsellor or religious/spiritual advisor if required.

Stage 3: Early stage care - At this stage we focus on enabling you to maintain your independence. You may need specialist equipment, so we will work with other medical professionals involved in your care to ensure you have everything you need.

Stage 4: Late stage care - During this stage discussions will take place regarding your end of life care and wishes. Our team will help to carry out any wishes you may have, whilst ensuring that you are as comfortable as possible.

Stage 5: Supporting loved ones - Stage 5 is when medical professionals involved in your care source and offer bereavement support to your loved ones.

  • Absolutely, we will work closely with any other medical professionals that are involved with your care, such as GPs, district nurses and specialist consultants, to make sure that you have the right support in place.
